Thank you for considering Ortho-Tec Medical, Inc. as your provider for Therapeutic Footwear. Please follow these instructions so that we can expedite the process of getting your new shoes.

1. Take the attached Statement of Certifying Physician Form to your Medical Doctor (MD or DO) - the one who is treating you for your diabetes. The form must be fully completed and signed.

2. Ask your MD or podiatrist to provide you with a detailed written prescription for
   
   
   "One (1) pair of Diabetic Shoes and
   three (3) pairs of multi-density inserts"

   
   Please remember that this prescription is only valid for 90 days from the date it is signed.

3. Ask your MD to provide you with a copy of your visit note . You must have documentation of an in-person office visit with the doctor that is treating you for your diabetes within 6 months of receiving your new shoes.
4. Once all the paperwork is complete, please contact our office. We will need all of your insurance information. Our billing specialists will verify your coverage, deductible and co-pays with your insurance companies. We will contact you if we have any questions. Once all of your paperwork is received, your new shoes will be ordered.
5. We will contact you to schedule an appointment to come to your home to deliver your new shoes, make sure they fit you properly and instruct you on their care and use.
6. Once you have received your new shoes, we will bill your insurance company for you

8. Remember to mark your calendar for January 1st. Most insurance renew this benefit each calendar year.
